- Security: All telehealth sessions and records are encrypted and stored securely.

Telehealth Consent Form:
This is available and required of all patients to sign upon scheduling their appointments.
CONSENT: You have chosen to receive care utilizing telemedicine. Telemedicine enables healthcare providers at a different location to provide safe, effective, and convenient care through the use of technology. Your privacy and personal information is valued. Oregon TeleMed utilizes HIPAA-compliant tools and technology to keep your information safe and confidential. As with any healthcare service, there are risks associated with the use of telemedicine, including equipment failure, poor image resolution, and information security issues. You also understand that you cannot be physically examined and that you may need to come to a clinic or ED to complete the assessment or do further tests. You also consent to receive communication about your appointment via text messaging, email, and/or phone calls.

- Accessible technology and materials for telehealth visits. Forms, consent documents, and educational materials are available in alternative formats (large print, Braille, or accessible electronic formats) upon request.

Controlled Substances Policy
Oregon Telemed complies with all Oregon state and federal regulations for safe prescribing. Our providers carefully evaluate each patient and only prescribe medications when medically appropriate and legally permitted.
- We do not prescribe opioids, narcotic pain medications, or benzodiazepines (such as Xanax, Valium, or Ativan) through telehealth.
- If you need these medications, we will refer you to an in-person provider who can safely manage your care.
- Stimulant medications (such as those for ADHD) and other controlled substances may require additional documentation, monitoring, and follow-up visits under Oregon law.
Safe Prescribing Practices:
- PDMP Check: We review the Oregon Prescription Drug Monitoring Program (PDMP) before prescribing controlled substances.
- Medical Documentation: A comprehensive medical history, current medication list, and diagnosis are required.
- Follow-Up Visits: Regular follow-ups may be required to ensure medications are effective and used safely.
- Monitoring: For certain medications, additional steps such as urine drug screens or coordination with other providers may be required.
- Refills: Controlled substance refills are closely monitored and may require a new visit, per state law.
How to Pay
Pricing: We bill insurance in Lane and Deschutes counties. Self-pay patients can access care at a discounted rate of $125 per visit when paid in cash.
Pay a Bill: If insured, your insurance will be billed directly. If self-pay, we will send you a statement. Questions? Call us at 541-726-4510.

- Translation Services: Support is available in your preferred language—please let us know your needs.
- Emergency Care vs. Urgent Care: Call 911 for emergencies. For non-critical concerns, use telehealth or urgent care.
- Birth Control: Confidential, judgment-free services for your birth control needs. Our providers will walk you through your options, answer your questions, and make sure you feel confident in your choice. Same-day prescriptions are available.
- GLP-1 Treatments: Coming soon for weight management—covered by some insurance or available for cash pay. In the meantime, please email us if you are interested in receiving this treatment.
- Mental Health: Telehealth counseling and psychiatric services launching in 2026.
